1. Insurance will only cover it if it's medically necessary (deviated septum, for example). The cost varies from doctor to doctor, Call around.
2. Insurance could pay if it is Medically necessary opposed to cosmetically. Just have to have a good medical reason why. Usually your doctor can come up with a good reason.

I just did a lot of research on this subject and the only way to get insurance to pay for it is if something tragic happened to your nose as a result of an accident or misfortune or if you were born with a congenital condition.
