What are the usual requirements in order for your insurance company to pay for weight loss surgery?

- Generally, your weight condition has to be some threat to your health. Are you really overweight? Heart conditions? Breathing problems? Mobility issues?
- u have to be obese and be more than 100 pounds over ,, call ur insurance to be for sure
- More than 100 pounds over weight. A year of counseling. Proof that you have tried other things. I helps if you have documentation of being overweight since childhood. If you get denied you have to appeal. Most are not approved on the first try.
- Lots and lots or requirements and they vary greatly between insurance companies. Best way to start if find who is allowed on your insurance and go to meetings with that physician. Most have informational meetings in groups for information. Then when you find one you are comfortable with, the office staff will help you get started on the paper work and all that. My insurance requires 6 month of wait plus classes, etc. Decide who first and then work from there. If the office will not help you then move to another as that is a sign that they are not really into it. I worked in a hospital with a lot of these surgeries and the doctor is the key to success. Some are just doing a few, some do it as their whole practice. Make sure they do enough to be a specialist. Search on line for your insurance and then search the individual doctors' sites and then go from there.
- My friend had gastirc bypass surgery. Her insurance company make her go to a shrink and make her family doctor sign a paper saying she has been on an approved diet through her doctor for at least 2 years and not has any success in losing weight.
- BMI of 40 or more (or 35 or more with comorbidities) health problems like: diabetes, high blood pressure, sleep apnea, high cholesterol, etc. Then to get it approved by insurance, you usually need: BMI and a physical cardio/pulmonary clearance psych evaluation diet history and perhaps a 6month doctor supervised diet.
